You can support them by talking to them, by making sure they have what they need to communicate with you, that you have any information they might need.
So you could start your reflective account by saying e.g.
I had noticed that resident x had not been very happy for a few days. i spoke to my manager and it was agreed that I would make time to talk to resident to find out if anything was wrong and also to use the opportunity that the service she was receiving was appropriate. I asked
